The breach was first detected on November 9, 2023, and it was determined that unauthorized individuals had accessed and encrypted certain systems on or before September 27, 2023[1][4][7][13][19][21].

The compromised data included a range of sensitive patient information such as names, mailing addresses, dates of birth, Social Security numbers, insurance information, diagnosis and treatment information, and other medical or billing records. Some guarantor information was also exposed[1][7].

Azura Vascular Care has responded to the incident by offering credit monitoring and fraud resolution services to individuals whose Social Security numbers were exposed. They have also engaged cybersecurity experts to assist with the investigation and have notified the HHS’ Office for Civil Rights about the incident[1][7].

The company operates over 70 outpatient clinics and ambulatory surgery centers across 25 states and Puerto Rico, providing minimally invasive treatment options for various vascular conditions[2][10][11][14].
