The guide
What to do, in order
Understand the decision before drafting
Work from the denial notice and the documents for your actual plan. Record the stated reason, claim or authorization reference, the date of the notice, and the listed review instructions. Ask the plan to clarify anything missing or unclear.
Build a document trail
Keep the original notice, relevant plan language, supporting records, correspondence, and delivery confirmations together. Your treating team can help identify relevant clinical documentation. This guide does not determine medical necessity, coverage, or your likelihood of success.
Keep the stated reason intact
Copy the notice’s explanation and codes, then ask what each means for your request. Avoid assuming that a billing error, missing authorization, a coverage exclusion, and a medical-necessity decision require the same response.
Your checklist
Your planning checklist
- 01
Copy the denial reason and reference number exactly from the notice.
- 02
Confirm the submission address, required form, deadline, and any urgent-review procedure with the plan.
- 03
Identify supporting documents and ask the treating team which information is relevant.
- 04
Keep a copy of the submission and proof of delivery, then record the plan’s response.
We send the request, follow up with the provider, and deliver the records to you. Your provider decides how fast they come back; the law gives them up to 30 days, and we post every verified update.
