The 21st Century Cures Act is a landmark piece of legislation designed to promote patient access and health information interoperability. A key part of this law is the Information Blocking Provision, which expressly forbids healthcare providers from knowingly interfering with your access to your own electronic health information (EHI).

This rule applies to nearly all clinical notes, including consultation notes, history and physicals, and discharge summaries. The only major exception is for "psychotherapy notes," which are kept separate from the main medical record.
