Indian Health Service

The Indian Health Service Resource and Patient Management System (RPMS) is a comprehensive, federally-developed electronic health record system specifically designed to serve American Indian and Alaska Native populations through the Indian Health Service, tribal health programs, and urban Indian health organizations. As a unique government-operated health IT platform, RPMS has been in continuous development and refinement since the 1980s, evolving from earlier Indian Health Service clinical systems.

RPMS distinguishes itself as a fully integrated health information system purpose-built for the federal Indian health system, addressing the specific cultural, clinical, and administrative requirements of serving over 2.6 million American Indians and Alaska Natives across more than 600 facilities. Unlike commercial EHR vendors, RPMS is provided at no direct cost to eligible facilities and includes comprehensive support through the IHS Office of Information Technology.

The system encompasses a full suite of clinical and administrative applications including ambulatory care documentation, pharmacy management, dental records, laboratory integration, radiology information systems, immunization tracking, and behavioral health modules. RPMS operates on a client-server architecture using the VA FileMan database management system, sharing technological heritage with the Veterans Health Administration's VistA system. The platform supports both centralized and distributed deployment models, with the IHS National Data Center providing hosting services alongside locally-managed instances.

RPMS serves facilities ranging from small rural health centers and clinics to larger hospitals and ambulatory care centers operated by IHS, tribes, and urban Indian organizations. The system has achieved ONC 2015 Edition certification, enabling participation in federal meaningful use and quality reporting programs. Key strengths include specialized modules for population health management, diabetes tracking, immunization registries, and contract health services—reflecting the chronic disease burden and unique reimbursement mechanisms within Indian health.

The platform supports interoperability through Direct Secure Messaging, eHealth Exchange connectivity, and Surescripts e-prescribing integration. Recent modernization efforts have focused on FHIR API development, enhanced patient portals, and improved user interfaces while maintaining the extensive clinical functionality developed over decades. As a government system, RPMS development prioritizes stability, security, and alignment with federal Indian health policy rather than commercial market trends.
