This map shows which EHR systems can actually exchange patient data with each other, labs, pharmacies, and clearinghouses — and through which standards. Click any node to see its connections.
How EMR Interoperability Works
Interoperability determines whether your EHR can exchange patient data with referring providers, labs, and pharmacies without manual intervention. The U.S. healthcare system runs on these standards and networks:
- HL7 FHIR R4 -- The modern API-based standard for exchanging healthcare data. Required for ONC certification and 21st Century Cures Act compliance.
- CommonWell Health Alliance — A vendor-neutral network enabling record location and retrieval. Members include Cerner, athenahealth, MEDITECH, and others. Epic is notably NOT a CommonWell member.
- Carequality — A trust framework that enables cross-network data exchange. Epic IS a Carequality member, which bridges to CommonWell.
- Direct Messaging — Secure email-like messaging for referrals and care transitions, managed by DirectTrust.
- Surescripts -- The nationwide e-prescribing network connecting virtually all EHRs to pharmacies.

CommonWell vs Carequality: What Is the Difference?
CommonWell Health Alliance
- What: Vendor-led health data exchange network founded by Cerner, athenahealth, and others.
- Members: Cerner, athenahealth, MEDITECH, Allscripts, NextGen, eClinicalWorks, CPSI, Greenway.
- Notable absence: Epic is NOT a member. Epic uses its own Care Everywhere network instead.
- Key capability: Record locator service that finds patient records across member organizations.
Carequality
- What: Trust framework enabling cross-network data exchange. Operated by The Sequoia Project.
- Members: Epic, Cerner, athenahealth, MEDITECH, Allscripts, NextGen, eClinicalWorks, Greenway, and many HIEs.
- Key advantage: Bridges Epic's Care Everywhere to CommonWell, enabling cross-network data exchange.
- Scope: Over 70% of US hospital beds are connected through Carequality participants.
Bottom line: If your practice runs on Epic, you can exchange data with CommonWell members through the Carequality bridge. But bridged connections tend to be slower and less reliable than native network-to-network exchange. Factor this into your evaluation.
